ID Clinic

To begin the process of changing your identification/legal documents you have to complete a change of name application through the registry. (Link below.) 


For your legal name change application you will need all your original IDs/documents as well as: 

  • A Criminal Record Check 
  • Electronic Fingerprint Confirmation Letter 


(Youths under 12 can have their information amended by parents without a record check or fingerprinting, youths over 12 will still require fingerprinting even with the application done by a guardian.) 


Once the application has been submitted and approved, the applicant will receive both a change of name certificate, and a birth certificate. (If you were born outside of Alberta, you need to contact your province for the birth certificate. If you were born outside of Canada, you need to process the birth certificate through your country of origin.)

Most of these applications or requirements have to be processed in person at the registry or RCMP office.  Once the change of name is complete you can change your photo ID and health care card information at the registry. (Not online.) 


Notes on changing your "sex indicator": 

  • Must be completed through Vital Statistics or through a registry agent office.
  • Application fee of 20$
  • Will need a signature from a notary or commissioner for oaths.
  • Will need an affidavit. 
  • Will have to forfeit existing birth certificate.

*At this time, Wood Buffalo Pride is unable to assist with application fees. 

*Note: the RCMP will ask for a certified cheque for fingerprinting services. Local RCMP only offers Fingerprinting Services at the Timberlea Location from 9 am to 11:45 am and 2pm to 3:45 pm.
